set up your first appointment
Welcome to Drs. Black and Benton Pediatrics!
The first time you call we are happy to answer any questions you have and will ask a few of you as well to get your child set up in our computer system. Here's what you can expect.
Congratulations on the upcoming arrival of your little one! We encourage you to get set up in our system before the big day so you can focus on one another once your little one is here. Once your baby is born, call us to set up your first appointment. Most hospitals require you to make this appointment before discharge, and we request you bring your hospital discharge summary to your first appointment with us.
Call anytime to get set up in our system and schedule your first appointment. It is simplest if your first appointment with us is a yearly physical, but it can be a sick visit if needed. In either case, knowing the month/year of your last physical is helpful but not required.
We recommend requesting a transfer of records from your previous pediatrician two weeks before your appointment. You can download the form here and fax it to us or come by the office to fill one out in person. We will take care of the rest.
about our office and in-house lab
Well and Sick Waiting Rooms
It is important to us to maintain two waiting rooms for our patients: one well, one sick. These designations enable us to distance patients as well as protect our infants and others whose immune systems are still building or suppressed. Patients here for a physical, med check, or conference visit may sit in the well room, while all others should wait in the sick waiting room. **Due to novel COVID-19, our waiting rooms are closed until further notice. Instead, we ask that parents call from the car when they arrive in order to be directed regarding which exam room to go directly to. **
In-House Labs and Immunizations
We perform standard labs in-house, notifying you of results as soon as possible within the time frame of the test.
Immunizations are an important part of pediatric care, one that we take with the utmost responsibility. Therefore, we stock all required immunizations in-house, so you will never need to make a trip to the Health Department.
Free and Easy Parking
We have convenient parking in front of our building at 4741 N Broadway. There are two entrances to the lot for your convenience: one from Broadway and one from Adair.
We have a ramp that leads directly to our door. It is located at the center of the building by the outdoor fountain.
Small and Privately Owned
Because we are a privately owned practice and not part of a larger network we enjoy autonomy as keepers of our patients' medical records. This means that should you require a piece of information, copy of your record, or request for a form or fax, we take care of all of it in-house in a matter of business days and do not have to climb bureaucratic ladders to serve your request.
